Good Faith Estimate

Right to Receive a Good Faith Estimate of Expected Charges

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • If you receive a GFE from us please make sure to save a copy or take a picture of it. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises

At The Vitality Center, we strive to assist you with your insurance benefits and billing inquiries. It's important to note, however, that the benefit information we provide prior to submitting a claim is an estimate, not a guarantee of payment.

Key Points to Remember:

  • Estimates Aren't Final: The estimated coverage we provide is based on preliminary information. Your actual benefits will be determined by your insurance provider once your claim is processed.

  • Engage with Your Insurer: We encourage you to contact your insurance company to fully understand your coverage. This step is crucial for being informed about any deductibles, co-payments, or co-insurance that may affect your out-of-pocket expenses.

  • Assistance from Us: While we can't predict the outcome of your claim, our team is ready to help you with any questions you may have about contacting your insurance provider.

What You Can Do:

  1. Reach Out to Your Insurer: Verify your coverage details directly with your insurance by calling the number on your insurance card.

  2. Inform Us of Changes: Let us know if there are updates to your coverage that may influence your services with us.

  3. Ask for Help: If you have any concerns or need clarification, we're here to support you.

We at The Vitality Center are committed to making your experience as clear and supportive as possible. Understanding your insurance benefits is a key step in managing your healthcare effectively.
